First Visit
We are happy you chose Ascend Chiropractic! When first visiting, there’s a few things to expect.

Intake forms must be filled out before being seen by Dr. Krantz. You are welcome to download and print this form at home to fill out and bring in. If you’re unable to fill out the paperwork prior to your appointment, we have a copy for you at the office! Please arrive 10-15 minutes early for your appointment to fill out the form. We have separate forms for adults, children (1-18 years), and infants (0-12 months).

Once your intake paperwork is completed, it's time for your initial exam. Dr. Krantz will carefully review your health history, conduct a thorough examination, and perform a neurological scan to assess your overall health and well-being. Throughout the process, Dr. Krantz will ensure that you feel informed and empowered with your personal health information. If you have any questions at any point during the exam, please don't hesitate to ask. Your understanding and comfort are our top priorities as we work together towards optimizing your health.
